Registration Information for 2025-26

Registration information for 2025-26 is provided below.

Registration:

• Levels k,1,3,4,5,6  $45: first child with each additional child $30
• Sacramental years: Levels 2, 7 (confirmation 1), and 8 (confirmation 2) $60

After September 1 (considered late registration):

• Levels k,1,3,4,5,6  $50: first child with each additional child $35
• Sacramental years: Levels 2, 7 (confirmation 1), and 8 (confirmation 2) $65

Please drop of registration fees during Mass (collection baskets) or at the parish offices.
PLEASE make sure the envelops are CLEARLY MARKED, "Faith Formation".

Please provide a copy of the baptismal certificate if the student was not baptized at Our Lady Queen of Martyrs, Sacred Heart, Holy Family, or Holy Trinity Parish.
